Immigration Medical Physical Examination

At Orlando 420 Doctor, we are officially approved to provide the Immigration and Naturalization Service (INS) Form 693 immigration physical medical examination. Our services assist aliens in seeking adjustment of status or meeting examination requirements for permanent residency.

MEDICAL EXAM

In accordance with the United States immigration law, immigrant visa applicants are required to obtain specific vaccinations before receiving an immigrant visa. Most applicants will undergo 2-3 medical visits as part of the process. During the initial visit, the physician will conduct the medical examination and administer the Tuberculosis skin test. Additionally, the applicant will be directed to a medical lab for a blood test to check for HIV/Syphilis and other necessary tests. If needed, the physician may also administer immunization shots and vaccines during this visit.

On the second visit, the physician will review the Tuberculosis test results to determine if any further examinations, such as chest X-Rays, are necessary. During the final visit, the physician will review the results of the HIV/Syphilis and other blood tests. Once all tests are completed, the physician will proceed to fill out the I-693 form, sealing it along with the medical results in an envelope.

WHAT TO BRING TO THE EXAM

For your appointment, please ensure you bring the following documents:

Government-Issued Photo Identification:

A valid passport or driver’s license will serve as suitable identification. If the applicant is 14 years old or younger, additional identification showing their name, date and place of birth, and parent's full name is necessary. Acceptable forms of identification include the birth certificate (with an English translation) or an affidavit.

Vaccination or Immunization Record:

Please bring along your vaccination or immunization record, which should include details of DT, DTP, DTaP, Td, Tdap, OPV, IPV, MMR, Hib, hepatitis A, hepatitis B, varicella, pneumococcal influenza, rotavirus, and meningococcal disease vaccinations.
